It addresses the primary causes of readmission by proactively ensuring that the patient is safe and comfortable at home immediately after discharge, and that they keep the follow-up doctor appointment. Hospital to Home care transition does not replace skilled home health services, but does provide the critical support services that can prevent readmission to the hospital. In the first visit, immediately after discharge, the caregiver can ready the house for client return, pick client up at hospital and return to home, get the client comfortable, and provide non-medical services such as personal care, meal preparation and housekeeping. Our trained caregivers can offer assistance with discharge instructions, keeping an eye out for warning symptoms, and observing the client’s health and progress. In the continuum of care program, which is timed from discharge to the day of the follow up doctor appointment, the caregiver can help the client call their primary care physician to schedule the follow-up visit, help dress/prepare for the appointment, review the questions the patient wants to discuss with the doctor and provide transportation to and from the physician’s office.
